The Belgian suffered an ankle injury in the 2-0 win at Tottenham on Tuesday night, forced off in the second half as a result.
The Premier League champions head into the final day of the season against the Hammers knowing a victory will secure them a fourth straight title, but they could be without the influential midfielder.
Speaking at a press conference on Friday, Guardiola said of De Bruyne: ‘We will see today. Yesterday he was off, today we train.’
The 32-year-old said after the game in north London that he felt he was ‘alright’ despite the painful knock.
‘I think I’m alright,’ he said. ‘The place in the Achilles is really sore. It felt a little bit like a knife in the moment I had the impact but it’s an impact [injury] so it’s not bad, I’ll be alright.’
Manchester City will certainly be without first choice goalkeeper Ederson on Sunday after suffering a small fracture of his eye socket in a collision with Cristian Romero at Tottenham.
